A school admission form is the first official document a prospective student's family completes. It captures the information the school needs to assess eligibility, allocate a class, set up the student's records, and make initial contact with the family. A well-designed admission form reduces back-and-forth with parents by collecting everything needed in one submission.

When to use: Use at the start of every admissions cycle, before any student is enrolled or assessed. Collect one form per applicant.

Best Practices

Collect birth certificate number or a copy — this prevents name discrepancies when registering students for national examinations such as BECE or WASSCE.

Always capture two parent/guardian phone numbers. If the primary number is unreachable in an emergency, having a second contact prevents dangerous delays.

Collect the parent email at admission — even if the school doesn't use email heavily now, it is needed to activate the Parent Portal when the school digitalises.

Use the "How did you hear about us" field to track which marketing channels (referrals, social media, church, signage) drive your admissions — this data is rarely collected but extremely valuable.
