A staff performance evaluation form provides a structured framework for assessing teaching and non-teaching staff performance at the end of each term or year. Regular evaluations help school leadership identify top performers for recognition and advancement, identify staff who need additional support or training, and maintain documented evidence of performance for employment decisions. Schools that evaluate informally or not at all struggle to justify promotion, salary, or dismissal decisions.

Share the evaluation criteria with staff at the beginning of the year, not just at evaluation time. Staff who know in advance what they are being assessed on can actively work towards the criteria, which is the entire purpose of a performance framework.
Student academic outcomes should be benchmarked against the class's starting point, not just final scores. A teacher who brings a weak cohort from 42% to 61% has outperformed a teacher who maintains a strong cohort at 68%.
The "Agreed Development Plan" section is the most important part of the form — yet most schools leave it blank. A development plan without specific actions, timelines, and support commitments is just an aspiration.
